*All sections,
calculations, and customer’s credit card billing address
must be completed in this form before tickets can be issued.
*A signature from the authorized signer must still be
obtained for corporate cards.
*Delivery charges will only be charged once.
*Air Travel and Tours will usually accepts Visa, Master
Card, American Express and Discover, other credit cards and
some destinations are subject to approval by airline. We do
not accept Debit Cards or Third Party credit cards. We will
obtain the approval on the credit card.
*Air Travel and Tours may appear on the cardholder’s
statement.
*Note: In addition to any Air Travel and airline penalties,
4% of total charge may also be non-refundable on
cancellations.

*Required documents: record locator, a copy of itinerary, a
photocopy of cardholder’s passport or drivers license, and a
